If you file a request for the notification of purpose of use according to Article 24, Clause 2 of Japan's Personal Information Protection Act (hereafter referred to as "the Act"), or disclosure according to Article 25, Clause 1 of the Act, for the security of personal information, we require you to provide identification. If a representative makes the request in your place, identification is required for both you and your representative. If you are 14 years of age or under, your request must be made by a legal representative. See below for information on each identification document.
You must provide one of the documents below, which must not be expired. (Note: Copies made by printing images taken by a digital camera or scanner are invalid.)
Note: If you have changed your address, make sure you have completed the procedure for changing your address, and then attach a copy of the reverse side of your license.
Note: "B Type" is required, which contains your photo, name, date of birth, and current address. If you have changed your address within the same city, ward, town, or village, you must attach a copy of the reverse side of your Basic Resident Register card.
Copies of both the page containing your photo and the "information on bearer" page (which contains your name, address, and other information) are required.
Note: Make sure that your current address is recorded in the address field.
Provide the following documents, within 3 months of being issued:
An official copy of the family register, official copy of part of the family register, or documentation proving the representative's relationship to you (the requester)

A power of attorney (original copy) signed or affixed with a seal by you (the request)
Provide one of the following: Transcript of register, Extract from register, Certificate of All Present Matters or Certificate of Partial Present Matters (Note: Only documents issued up to 3 months ago are valid.)
